Viagra and Heart Health: How Sildenafil Affects the Cardiovascular System

Four years earlier, while delivering her first child, she had severe PAH — her right heart pressure was five times the upper normal limit. After aggressive treatment, her previously enlarged right heart returned to normal size, and right heart pressure dropped to twice the normal upper limit.

The key difference: the woman had “primary” PAH (not caused by another disease), while the man had “secondary” PAH caused by chronic lung disease. Both were treated with an ED medication.

How ED Medication Affects the Heart

Currently available ED medications include Sildenafil (Viagra), Tadalafil (Cialis), and Vardenafil (Levitra). These work by inhibiting the PDE5 enzyme, which normally breaks down cGMP — a chemical that dilates blood vessels. By inhibiting PDE5, cGMP levels increase, blood vessels dilate, and right heart pressure decreases.

Since PDE5 is mainly found in pulmonary artery walls and the penis, ED medications primarily affect these two organs. Studies show that PAH patients treated with ED medication experience significantly improved exercise capacity and reduced right heart pressure.

In the male patient’s case, after taking ED medication for just a few days, his right heart pressure dropped from three times the normal upper limit to twice the normal limit. Some studies suggest these medications may also slow PAH disease progression.

Beyond the Heart

Interestingly, ED medications have shown benefits for other conditions too. Certain people who engage in high-altitude climbing may experience High Altitude Pulmonary Edema (HAPE) — a life-threatening complication caused by abnormal pulmonary artery reaction to low oxygen levels at altitude — and ED medications (especially Viagra) have been shown to effectively treat and slow HAPE progression.

Important Precautions

Smokers with lung disease and breathing difficulties should understand ED medication side effects before taking them. Common side effects include headache, facial flushing, and nasal congestion. Viagra users may also experience blue-tinted vision. Rare but serious side effects include vision damage and sudden hearing loss.

Not all smokers with lung disease will benefit from Viagra. While some patients respond well, others may only experience minor symptom relief. Results vary significantly between individuals.
